Hey - I have recently done some research on a company called Fonality, www.fonality.com and they used to be in the Residential VOIP game - but they've just released a PBX replacement based on Asterisk...they've implemented a VERY cool looking, easy to use GUI that controls alot of features...They're only charging $995 for the server with the software preloaded.
I don't think they've launched the product, PBXtra on their website yet - but I know they have customers using it....hope this helps...laidbacksoul

www.astium.nl is also asterisk based. Its sold with a management contract for backup and updates.

We have a free auto-installer for Debian Linux and Asterisk, for non-Linux pros that want to check Asterisk out. It basically installs your computer from scratch. It taked about 10 minutes to install and in the end you have an Asterisk station (without any configuration, but it will have a default configuration in the next version which will very soon come out).

Fonality-

If you want hidden fees and a semi hosted environment than fonality is the way to go- I on the otherhand prefer not to be married to my vendor...

If you want to be punished for growth and all the other things involved with a IP PBX implementation then you should give fonality a serious look

If not - check out switchvox... It is a great solution that is now owned by Digium, the corp sponsor and creators of Asterisk
