Phone Sometimes Not Ringing

System: KX-TDA200
VM: KX-TVA200
Phone: KX-T7633
Connection Type: PRI

I have a user who complains that her phone sometimes does not ring and instead goes straight to voicemail. She is sure the messages aren't coming in while she is already on the phone. She is a receptionist at a dental clinic and will have no messages, be at her desk for a while and not hear it ring, and suddenly have one or more messages. Timestamp indicates that the messages were received during the time the phone didn't ring. It is an intermittent problem- the phone usually works normally but doesn't ring on maybe 5% of the calls.

Things I've already tried:
Train user on use of DND/FWD (she never changes this, always leaves it on FWD)
Check FWD/DND settings in Maintenance Console
Replace phone
Move extension to new port on PBX
Delete mailbox
Assign new mailbox number
Reboot PBX and VM sytems

I believe the user, and have seen it happen while in her office. I'm running out of ideas. No other users on the system are having this issue.

being a pri is any did's pointed to the VM that the customer is ringing instead of main number.
is the call coming in through the AA to reception or is it ringing straight in to her phone?
is the call coming in to a group or to the phone?

stick a printer or a pc with terminal on the system and see what is hapening with the call, if through AA run it on the vm unit

double ck fwd on ext & co settings I have set through the keypad then verified
and went in through with pc console and have those settings not be the case

you also need to make a bunch of test calls and watch the digit stream coming in
the clec switch may be misfiring digits....

Thanks for the replies. I still haven't figured it out. My replies to your questions are below.

The extension does have a DID. However the extension is primarily reached by dialing a main number that has Auto Attendant, then entering pressing "4" for dental clinic. It is not part of a group. It will be a little while before I can connect to the VM unit and check it.

I double checked the settings and didn't see anything out of place. Clec is a possibility, but I think it's more likely that the issue is VM related since I'm now told that almost all of their calls come through the auto attendant.
