Headset button not working

I am having an issue on one particular NEC D-Term 16 button phone connecting to NEAX 2400 IPX. Yesterday the headset stopped working. I have replaced everything from the line cord all the way up to the headset. Incidently, we are using Plantronics M12 headsets, so I replaced the A/C adapter, all cords, headset, amp box, etc. I also replaced the phone, reset the heasset key in AKYD, moved it to a different button, completely deleted AKYD data and recreated, reset position data in ACD, and removed agent from ACD and recreated..still no luck. Oh yeah, I adjusted the settings on the amp boxes, too.

Any ideas?

When you depress the head set button does the line key go off hook?

You stated this is an ACD position, so depress the head set key, then the PBX line, does it light the line key? That would tell you if the head set key is working correctly.

Next you would have to look at the connections and parts, which it looks like you have already.

First just make sure the AKYD is working correctly. Also, is this a 16 telephone, how is the TP set in AKYD and what about line key pattern in ADKS (soft key assignment).
