i need Meridian PBX help

I have a opt11c with vmail, I interface my property mngment system via a rsm board. I just purchased a 4 port breakout cable (ntdk58aa) I connected it up and can't get it to work, there was no documentation as to what cable connected where. Does anyone know what cable connects to what?

RSM board cabling

yes, it has been working for some time we just decided to clean up the cabling, and ordered the 4 port breakout cable.

If ports 0,1,2 work and port 3 doesn't then I would go to LD 22 and print the ADAN and hopefully the previous install tech DES'd the TTY properly, I would try and use that as a guide.
