I my experience the only way to get this fixed is set up est time with a PRI tech at the co and have him monitor your transmission to verify what they are really seeing. Here in Chi-town there are probably only two folks in provisioning that know what tey are doing the rest always blame something else, as they don't understand what they are seeing.
