Disappearing Transfer Button

We have a 61C, using 3904 phones. Occasionally I'll get a call from a user saying that they had a call on one of their lines and they had no option to transfer it because the TRANSFER button was not showing. But it does show on subsequent calls.

This has happened often enough to enough different users that I believe it to be a real issue and not user error. Any ideas what might be causing this? Are there certain kinds of calls that the system will not allow to be transferred? Or is there a limit to the number of times any one call can be tranferred around? I can't find a common criteria in the instances that I've been called on.

Any help would be greatly appreciated.

Can you find a common link on the calls that the problem occurs on i.e, are they always internal or external, tie line calls, if external via a specific route?

Are alll the 3904's the same Rls and have the most up to date firmware?

Are all of your conference packs and channels working?

Is it and outside transfer? This could be a supervision/trunk signaling problem.
or is it transfers to another inside extension.

Also, do you have enough TDS/CONF loops to cover your users?

I have to deal with silly users almost every day (as I'm sure most of you do ), and I came across this a couple of times. On 3904 sets, the transfer button will only show if there is an active call on the set; if there's no call, or the call is already on hold, both the transfer and the conference buttons dissappear.
Have you doublechecked that those users complaining are not placing the call on hold before initiating the transfer?
Just a thought...
