Can someone help me understand the cause?

Don't see a fault code 000039 in the Rls 25 doc. What does it say in the Succession docs ?

What part of the Mid did it INI ?

Nothing in the 3.0 docs for

INI000 00000039

It took place in ld 60.

I would guess if we can't find the docs to explain the fault code, your vendor or Nortel would be the next step

The only place that I can even find a mention of 00000039 is under INI0100. Under that entry your error would be part of the DDSL pack failure. If your system is an 81 then this 39 would mean:
"DISKOS: Unable to start SL-1"

This does not sound like your issue or you would have a down system. Yes?

I agree with above, if you can't find this fault code listed then you may need to contact the next step up in the food chain.
