Patching Best Practice?

Just curious out there what is everyone's out there best practice when it comes to patching systems. I feel there is a wide varity of ideas about how often you should patch if you should patch and what patches you should use. We are working on adopting a policy where we patch quarterly but don't know if we should use patches that are 3 months old to patch or go with most recent patch. Obviously if you have a problem you take what they have but if you are running clean what do you do???? Would love to hear some of your ideas.....

I would say wait 2 months as sometimes patches need patching.
Nortel just released patch SU 21 which caused kaos!!
Here is the example:
SYMPTOMS: ISSUE: After application of BCM.R400.SU.System.021-200905 following programming may be defaulted or
lost:
- nnadmin password.
- Date & Time source
- "Accounts" settings
- "Security Policy" settings
- "Music Source"
- "Routing Table"
- "Scheduled Backup" setting
WORKAROUND: NONE, Restore affected systems and database segments/settings from system backup.

Otherwise only patch when you have an issue, it might fix the issue or you will need to install it anyway to get support from Nortel.

We have a sight we haven patched in 3 years and it works flawlessly.
