Check the wireing to the phone. Make sure if you are using a 66 punch down block that the bridging clips are in. Check the line cord from the jack to the phone. Take the phone to another 500 set and see if it is the phone or the jack that is the problem. Tone the jack back to your switch, make sure you are on the right pair of wires.
