Have you tried testing without the Vmail attached? I am wondering if it will still happen without the voicemail attached.
It also sounds like the vmail is doing a monitored or controlled transfer. The vmail holds the call and waits for an answer and then releases the call. But it sounds like the call stays on the vmail port and the vmail thinks it released the call, but is actually disconnecting the inside person, and hanging up on the caller.
