Which phone are you trouble shooting? One thing you may want to test is if the trouble exists every time, or just on two phones...
I HAVE run into similar issues, but after some Sherlock work we'd come to find that the trouble only existed btwn two phones, and the one we'd been trouble shooting (with HF) was NOT the troubled phone, but the handset of the OTHER phone was.
