You will never see a change in that column. The YES is an indicator that there are special instructions for the patch. It does not automatically mean a SYSLOAD or INI is requrired. It may be as simple as "If you load this patch, make sure patch XXXXXX is loaded as well."
Special Instructions indicate that there is some additional information that the user should know about when installing this particular patch. For example, here are the "Special Instructions" for MPLR20769 (your patch handle 32).
